We are in-network with HMAA, non-par Medicare.
FINANCIAL AGREEMENT
For Epik Chiro patrons:
One of our goals for Epik Chiro in 2024 is accountability and goal setting. In order to provide equality, we are implementing the following January 1, 2024:
Any change of an appointment less than 24 hours from the scheduled date and time will incur a $50 charge to the scheduled patient.
The $50 fee must be paid before scheduling any future appointments.
The scheduled patient must call or email the office.
The patient that has a signed Consent form in their file (e.g. Guardian, Power of Attorney, Minor Consent), that patient and/or the authorized person can schedule a new appointment, reschedule, or cancel an appointment.
The staff will defer to the waitlist if there is any change to an appointment.
Appointment changes more than 24 hours from the scheduled time will not incur a $50 charge.
We will cancel an appointment if the patient arrives 9 minutes after the scheduled time and charge a $50 fee.
This will give equal opportunity to all patients for openings in the schedule, minimize the size of the waitlist, and stay compliant with HIPAA Patient Privacy laws.
